Thank you for your interest in having your child become a new patient at Urbana Pediatrics. Please visit the different sections of our website – our website has valuable information about our providers, our practice, and the services that we offer.

In order to continue to be able to provide high quality, timely medical care in a warm, friendly environment to our established patients, Urbana Pediatrics is closed to new patients except for newborns. We will continue to be able to care for our established patients who change insurances if we participate with their new insurance. 

Vaccine Policy Statement

  • We believe that vaccinating our patients is one of the most important things that we can do as medical providers because vaccines help prevent many serious and life-threatening illnesses.
  • We believe that vaccines are safe and effective and do not cause autism or other neurodevelopmental conditions.
  • We follow the American Academy of Pediatrics and the Center for Disease Control’s vaccination schedules and will not allow children who follow alternative vaccination schedules in our practice.
  • We will not allow unvaccinated children in our practice unless they have a true medical contraindication to receiving vaccines.
  • We know that there is a lot of information on the internet about vaccines. The following are some of the websites that we recommend visiting to obtain information about vaccines:
  • We will always discuss the vaccines that your child is due to receive at his or her visit and will provide you with Vaccine Information Sheets (VIS) to review. We are happy to answer any questions that you may have about the vaccines that your child is due to receive.

Please review the list of insurances that we participate with. If you have any questions about our insurance participation, please contact our office.

Although we see patients through age 21, if they are attending college or another type of school, we generally do not register new patients who are 17 years old or older.

Please complete the New Patient Registration Packet for each of your children and either email the completed packet to Tiffany@urbanapediatrics.com or you can drop the completed packet off at our office during our normal business hours.

Please complete the Request for Medical Records form and email the completed form to Tiffany@urbanapediatrics.com or you can drop the completed form off at our office during our normal business hours.  We will fax the medical records request to your child’s previous medical provider.
